• School Entry Requirements

    for all new enterers into the Brandywine School

    District

    Immunizations Required

    Number of doses required

      DTaP or DTP, Td  (Diptheria, Tetanus

      and Pertussis vaccine)

      5 or more doses  (unless 4th dose was given after the 4th

      birthday)

      IPV or OPV   (Polio vaccine)

      4 doses of IPV or OPV (unless the 3rd dose was given after

      the 4th birthday)

      Hepatitis B vaccine                  

      3 doses

      MMR (Measles, Mumps and Rubella

      Vaccine)

      2 doses

      Varicella (chicken pox vaccine)

      2 doses of Varicella or a written disease history by a licensed

      health care provider

      Tdap* (Grades 9-12 only)

      All students in grades 9-12 must have 1 dose of adult

      booster

      Meningococcal Vaccine*(Grades 9-12)

      All students in grades 9-12 must have 1 dose (required)

      The Delaware Division of Public Health strongly recommends

      1 dose at age 11-12 and a booster dose at age 16

    *ALL current students upon entering 9th grade must have  1 dose Tdap (adult booster) , 1 dose meningococcal vaccine and a physical exam within the previous two years of entering 9th grade.

     

    Additional Health Requirements

     

    Required

      Health Examination

      Must be current within the  two years prior to school entry

      AND upon entry to 9th Grade (required of current & new 9th

      grade students)

      Tuberculosis Screening

      Results of  tuberculin test or risk assessment completed

      within 12 months prior to school entry

      Lead Blood Screening *(KN or

      pre-school only)

      Documentation of test for children entering kindergarten or

      pre-school program

    Children who enter school at kindergarten or at age 5 or prior,

    shall be required to provide documentation of lead screening

     

    Children who enter school prior to age four (4) shall follow current

    Delaware Division of Public Health recommendations.
