Necessary Documents
Required Documents for Registration:
Please have the following documents ready when submitting your registration packet:
- Birth Certificate
- Parent/Guardian Photo ID
- Medical Information:
- Current immunization records
- Physical examination dated within the last 24 months.
- Two Proofs of Residency
- Provide either one document from Group A and one from Group B, or two documents from Group A

Copy of a recent month's mortgage statement (if the home was recently purchased and a mortgage statement has not yet been received, provide a copy of a settlement statement). |
Car registration or insurance policy, or |

Rental agreement showing legal parent/guardian/caregiver as an occupant |
Homeowner or rental insurance policy, or |

Utility bill - electric, water, propane/oil ONLY. (The service location must match the mailing address, the bill must be current, and it must be issued in the name of the parent/guardian.) |
Most current tax year documents, or |

Current year's sewer bill. |
Recent paycheck or paystub, or |

Current year's real estate tax receipt. |
Notarized letter from employer verifying employment and address, or |

Two current consecutive bank statements or |

Official US Postal Service change of address notification on forwarded mail (yellow label with new address must be attached to the envelope next to the old address). |

Official letter from a State of Delaware agency such as DFS, DHSS, Department of Labor, etc. |
If you are living in someone else’s home, both of the following affidavits must be completed:
- Owner/Renter Affidavit of Multiple Occupancy
- Affidavit of Multiple Occupancy
- Owner/Renter Affidavit of Multiple Occupancy (Spanish)
- Affidavit of Multiple Occupancy (Spanish)

Foreign Exchange Students:
Foreign exchange students must register through an organization approved by the Council on Standards for International Educational Travel (CSIET). Please ensure both the student and host family are working with a CSIET-certified program.
