  • Welcome to our new students and families and welcome back to our returning students and families!

    The main function of the school nurse is to keep students healthy, safe and ready to learn.  We are the sole medical professional in the building and are here to support our students’ physical, mental and emotional health.

    If your child needs to take any medications at school this year, please be sure to keep them in the original prescription bottles.  A permission form will need to be filled out for each medication.  Controlled medications must be brought in by a parent or guardian, they are not to be brought in by the student.  See attached permission form.

    If your child has an EpiPen, inhaler or other emergency medications, such as anti-seizure medications, please be sure these are brought to the nurse’s office at the beginning of the school year.  Please have your doctor send an updated action plan for allergies, asthma and seizure disorders to the school nurse each year.  It is important that the school nurse is made aware of any health problems your child may have.

    Proof of immunizations, a tuberculosis screening and a physical must be presented upon registration for school.  If your child wants to participate in sports, a DIAA physical form must be completed by your doctor.  This form is specific for sports and annual physical exam forms will not be accepted.  ~ This link will take to you to the DIAA 2020-2021 PPE Form that is required each year.  Please note that this is an updated form for this school year.

    Emergency cards must be filled out by a parent/guardian at the beginning of the school year. Emergency cards for 2020-2021 school year were sent home with Chromebook distribution in September.  It is vital to fill these forms out with the most accurate and up to date information, including phone numbers and pertinent medical history.  Permission for as needed medications, such as Tylenol, Ibuprofen, Tums and Benadryl, must be filled out on this form in order for your child to receive these medications. 

    Postural and gait screenings will be completed by the school nurse for 6th, 7th and 8th grades.  Vision and hearing screenings will be completed for 7th graders only.  Abnormalities will be reported to the parent/guardian as well as recommendations for follow up care.
