     Hello and welcome to Lancashire Elementary School!
     

    I am looking forward to sharing a healthy and productive school year with you!  Please do not hesitate to contact me with questions or concerns about your child's health.  I am here to ensure the students are healthy and ready to learn! I welcome any and all updates on your child's health!

     
    Below is a helpful checklist of the health documentation that is required by law when your child starts school in a Delaware Public School.
    1) A current and up-to-date immunization record    Click to view Immunization Requirements
    2) A physical exam completed within the last 2 years
    3) A lead test (PreK and KN only)
    4) A TB Risk Assessment or skin test completed within the last 12 months
    5) A completed Emergency Card is required upon starting school. 


    Please provide any new health information, such as immunizations or physicals to the School Nurse to stay up-to-date with state requirements, as well as new contact phone numbers that may change throughout the school year.                   
    MedicationIf your child requires medicine at school, please complete the required permission forms with the School Nurse.  Prescribed medications need to be in the original container with the pharmacy label on it and brought in by an adult.  Medication will NOT be sent home with a student.

    Food Allergies: Life threatening allergies do exist within this school. Our goal is to keep children safe. Please discuss snack options for parties with the teacher before bringing them into the school.                                                                                                                                  Please feel free to contact me if you have any questions.
