If you are residing with someone else, please follow these guidelines to provide verification of address at the time of registration.
* If living in the residence of another person (not a rental property)
The residence must be the primary residence of the homeowner. The owner must accompany the parent/guardian/caregiver at the time of registration and present a Delaware driver's license or state ID in addition to the required documentation as listed in the table here. Both parties must complete the Affidavit of Residency stipulating that the registering parent/guardian/caregiver lives in the residence. (Declaración Jurada) Within 30 days, the person registering the child must present two items from Group B as listed in the table here.
* If living in a rental residence of another person
The residence must be the primary residence of the leaseholder. The leaseholder must accompany the parent/guardian/caregiver at the time of registration and present a Delaware driver's license or state ID in addition to the required documentation as listed in the table here. Handwritten leases and non-apartment complex leases must be verified by BSD personnel prior to enrollment. The registering adult and leaseholder have two options:
1. The leaseholder may add the names of the registering parent/guardian/caregiver and child(ren) to the lease agreement, or
2. If the names of the registering party and child(ren) are not on the lease agreement, the landlord must complete a notarized Residence Verification Form attesting to the fact that the parent/guardian/caregiver and child(ren) reside at that address. (Formulario de verificación de residencia)
In both instances, the parent/guardian/caregiver must present two items from Group B as listed in the table here.
